С 6.06.1960 — К-91. 15.09.1965-30.11.1967 модернизация в Северодвинске (СРЗ №891 «Звездочка») по проекту 629А под ракетный комплекс Д-4. 07-09.1968 в составе ЭОН-78 по Севморпути совершила межфлотский переход на Камчатку. 12.09.1968 перечислена в состав Тихоокеанского флота. С 25.07.1977 — Б-91.
